چکیده
Background: Iodide transport defect (ITD) is a dyshormonogenetic congenital hypothyroidism caused by sodium/iodide symporter (NIS) gene mutations. In the lactating mammary gland, iodide is concentrated by NIS, and iodine for thyroid hormone synthesis is thereby supplied to the infant in the breast milk. Case Description: A 34-year-old Japanese woman was diagnosed with ITD caused by a homozygous NIS gene mutation T354P. She had begun treatment of primary hypothyroidism with levothyroxine at the age of 5. In the lactating mammary gland, iodide is concentrated by NIS. What Does This Case Report Add? • The iodine concentration in an ITD patient’s breast milk is low. Possible iodine deficiency in the infant breast-fed by an ITD patient should be kept in mind.

متن کاملBreast milk iodine and perchlorate concentrations in lactating Boston-area women.
CONTEXT Breastfed infants rely on adequate maternal dietary iodine intake. OBJECTIVE Our objective was to measure breast milk iodine and perchlorate, an inhibitor of iodide transport into the thyroid and potentially into breast milk, in Boston-area women. PARTICIPANTS The study included 57 lactating healthy volunteers in the Boston area. MEASUREMENTS Breast milk iodine and perchlorate con...

Transport of iodine in the mammary gland into breast milk plays a central role in various fields of prevention of thyroid diseases. First, a sufficient content of iodine in the mother's milk is necessary for normal brain development in the breastfed child. This is attained by expression during lactation in the mammary gland of the sodium iodide symporter (NIS), also responsible for iodine trans...
متن کاملBreastfeeding and maternal and infant iodine nutrition.
OBJECTIVE The aim of this review is to explore information available regarding iodine secretion in milk, both mothers and infants iodine nutrition during breastfeeding and to make recommendations for appropriate iodine supplementation during lactation. DESIGN MEDLINE was queried for studies between 1960 and 2007 that included lactation and breastfeeding with iodine and iodine deficiency. Stud...
متن کاملComment on "Intake of iodine and perchlorate and excretion in human milk".
Perchlorate, thiocyanate, and iodine excretion in urine and milk of 13 breastfeeding women was investigated and the results were interpreted by a model of parallel/competitive transport of these species bythe sodium iodide symporter. For each species i, we assumed physiological homeostasis, where i(T,in) equals the corresponding total excretion in urine and milk (i(e,u) + i(e,m)). The fraction ...
